Schedule & Intake
You (or your caregiver) contact us or fill out the intake form online. We confirm eligibility, insurance, and home address in Greater Houston.

In-Home Assessment / Visit Day
Our clinician arrives at your home at a scheduled time, reviews your medical history, symptoms, vital signs, and concerns.

Diagnosis & Treatment
Based on evaluation, we provide appropriate care (medications, wound care, ordering labs, referrals).

Follow-Up & Oversight
We conduct follow-ups (in person or via telemedicine), coordinate with specialists, adjust the treatment plan as needed, and monitor your progress.

Continuous Support
We stay engaged—managing your care, handling prescription refills, ordering supplies, and responding to your changing needs.
