What is Care Plan Oversight?
Care Plan Oversight (CPO) is the ongoing supervision and management provided by a physician for patients receiving care through home health agencies or hospice programs, particularly those with complex or multidisciplinary medical needs. This oversight ensures that patients receive coordinated, high-quality care tailored to their unique conditions.
What CPO Involves:
Physicians performing CPO engage in a variety of activities to support patient care, including:
- Reviewing patient charts, reports, and treatment plans.
- Evaluating diagnostic studies that are not part of routine evaluation and management (E/M) services.
- Collaborating with healthcare professionals outside the physician’s practice who are involved in the patient’s care.
- Conducting team conferences to ensure coordinated treatment strategies.
- Discussing medication plans, including drug interactions (beyond routine prescription refills), with pharmacists.
- Coordinating care across multiple providers, ensuring proper implementation of complex treatment plans.
- Making and implementing necessary changes to a patient’s treatment plan to optimize outcomes.

CPO services are specific to home health and hospice patients and are not covered for patients residing in skilled nursing facilities (SNFs), nursing homes, or hospitals.
